How much muscle do you lose on Ozempic, Wegovy or Zepbound?

In the main trials, about 39% of the weight lost on semaglutide (Ozempic, Wegovy) and about 25% on tirzepatide (Mounjaro, Zepbound) was lean tissue, measured by DXA scan. Lean tissue includes water as well as muscle, so muscle alone is less than that. How much you lose depends mostly on two things you control: whether you lift weights and how much protein you eat.

How do I keep muscle while taking a GLP-1?

Lift weights two or three times a week, hard enough that the last two reps of each set are a struggle, and eat roughly 1.2 to 1.6 g of protein per kilogram of body weight a day. Then check it: if the weights you can lift keep falling while the scale drops, you are losing muscle along with fat.

How much protein should I eat on a GLP-1?

A common target is 1.2 to 1.6 g per kilogram of body weight a day, which is about 110 to 145 g for someone weighing 200 lb. Appetite drops on these medications, so most people fall short without planning for it. Put protein first on the plate and use a shake when meals are small.

What happens to my muscle when I stop taking it?

Studies of people who stop find that much of the weight tends to come back, and without training it can come back as more fat than muscle.
